Clarification (and spoilers) ahead.
At the start of the story, Ruka is biologically male. Later on in the story, he reveals that he has always wanted to be female, and through time shenanigans (and an urban legend that eating more vegetables while pregnant increases the chance of having a girl) they make it happen. In one ending of the game, Ruka stays a girl (Rukako) and gets together with the main character, but in the true ending and in the anime, they undo the time thing with MORE time things, and Ruka becomes a boy again. Assuming that the feelings remain across alternate timelines (which it's pretty safe to say is true, since feelings and memories have done so in several other instances) that means that poor Ruka really wants to be a girl, and has feelings for the main character, and has never told anyone, which is kind of sad.
