not totally a trap cuz is the spirit of a girl in a boys body so this are 2 souls in one body like run and ren from tolove-ru and not one soul with two bodies. but my question is where this come from cuz i have the complete series of ranma 1/2 and the quality isn't something like this mine is lower, and i though it was like that cuz it was an oldie anime.

OVA episode 13 I think.
Its much more recent than the others. Hence nipples with slightly more detail than ever appeared when the series actually aired.

theres no "two souls in one body", the story is that Ranma, during a training trip for martial arts fell into a cursed pond of "drowned girl" while his dad fell into the cursed pond of "drowned panda" so whenever he's soaked with cold water he turns into a girl, while hot water turns him back to a guy, his original form.
