He is not the last level 5, is still unknown perhaps the author drawing the image based on theories that may Aogami is the last level 5.

I think at one point when touma wanted to ask for help, the first person that came into his mind, out of all people, is this guy(that was before the memory loss I think)

Most people theorised Pierce Aogami is the mysterious Etsu Aihana.
Pierce's ability is unknown, and Etsu's were as mysterious as it is. One thing it is, that someone using Etsu's name has strong abilities.
Both blended to the surroundings.
Both had blue hair.
Touma said that nobody can mess with Pierce.
