The heart symbols is a common pictoral representation of moans/groans in anime/hentai based art. The fact that she's visibly crying -would- , imo, imply rape.

Based on other works from this artist, I think it's not so much rape as enjoyment of humiliation...they're "doing their duty", but they're humiliated, but they love it, etc...

The nature of the drawing is ambiguous. If you want it to be rape, then it's rape. Otherwise, it's consensual.
