Looks like a slapdash photoshop. Her face doesn't look all that much like Yozora's, though that may appear so because her body is a brick with almost no curves which torso is too long compared to her head.
Seems to me more like a not so well drawn (proportions are not a mere point of view) adult Yozora cosplaying with her younger days' school uniform than anything else.
