Anon4: Though a little girl may sometimes use it in that sense, a boy will normally instead only use onii-SAN to refer to a random young adult (though even then... that one is still kinda unlikely, a young boy will most often use nii-chan [unless he's like 3 or something..., in which case onii-san suddenly becomes the likely one]) and reserve onii-CHAN for someone much more personal and familial (or most likely just his brother), though they still often use onii-san or nii-san for that (another somewhat likely but not nearly as much: aniki).
