Something like "Whoops, wrong hole," follows from the previous picture. Basically, he's squeezing the enema bulb up her nose.
The fluid in most disposable enema syringes is glycerine. It is a thick, viscous sugar substitute and is a primary ingredient in soap bubble solution. In pure form it irritates mucous membranes, and thus is a laxative, a main component of s&m enemas, and would totally suck to get it in your nose.
