I don't know the canon of this series at all, so I can't say, but if they're implants, one would assume the character went to a plastic surgeon and got them (or got them through magical means, perhaps).

Or, the artist decided that's what they are, and they got there via artistic license.

Shouldn't this be tagged eyeshadow and not eyeliner? It's smudged off the sides and come up to the eyelid's defining line, like eyeshadow would.

Eyeliner is generally just near the lash line, and not usually down as far as it is here.

Arisu Hanabusu is indeed male. This if from "My Doll House". The male character crossdresses to model. No one knows that Arisu is actually male by the name of Kagosaka Mahiro . Those are breast implants that his doctor implanted in him so he could do a island swimsuit shoot for a magazine and not be outed as male for having no breasts, thus ruining is modeling career. The Mahiro, emits a potent pheromone that places any woman he has sex with under his "spell." He has quite a harem of beautiful girls all chasing after him and living in his big luxurious house. He spends most of the time living as a woman and having sex with the girls. There is a problem in that according to the doctor he left the implants in too long and now he must decide if he wants to live this way or become a woman. That's the general story in a nut shell.
