This is just a guess, Anon2, but I believe that the gauge on top of the apparatus measures heat, likely the heat produced by speaking; and yes, there is heat when you speak, since your jaw is moving and movement creations friction which creates heat. It seems likely that, if it got above a certain level, it would move the chain, turning the wheels, and pulling the trigger, killing her, hence the "Silence" tooled into the leather strap around the revolver's grip.

It's a heat gauge, but she's got her tongue to the metal to keep it heated. The idea is to keep her tongue outside of her mouth, so she can't speak. If she puts it in her mouth for too long, the gauge registers and the gun goes off. Notice that there's a wheel + chain for the pistol trigger. It notches slowly ... this lets her realize she's screwing up...by not keeping her tongue on the metal to heat it, it notches the wheel closer to the trigger little by little. Eventually it notches it one final time and *BLAM*.

An actually effective way of doing this would be to simply attach a string to her tongue and put it in place of the button chain section.
Or you could use a dead-man switch with a pneumatic actuator.

That garage or whatever looks closer to that of a pressure meter like for air compressors and the trigger for that gun would require a much more powerful force then that small device would be capable of producing. it looks more like a device meant to cause fear not to really kill
