It's referred to as "she" more than once in the show.

Given Kaoru's apparent age, it's unlikely that she's a woman unless she got a mastectomy.

She could be an honest-to-goodness transgender person (very rare in anime, I know) so while biologically male she is gendered female. The other people in the community look nice and accepting enough so they gender her as she wants to be gendered.
