No just because Kaine has a penis doesn't make her a boy. In canon, she is a female hermaphrodite. While expressing her female side may seem as important to her as it is to a transgendered woman. And while she suffered persecution in a similar way to a transgendered might, the two are not the same. Kaine is a woman on the inside, and biologically she is also female. She just has the extra part from a mutation in the replicant process. A transsexual woman, which is covered under the transgender umbrella term, is mentally a woman, but biologically a man.
The closer real world term to Kaine isn't transgender, but intersex. Of course, this translates to futanari on here.
