The device is a rebreathing bag. Ordinarily, they're made so that when you exhale, it inflates the bag, vents some of your exhaled breath out via a valve or perforations, and then you inhale mostly what you exhaled, but you get some fresh air in. Properly set up, there's not much danger. You just experience a high and a mild to moderate sense of desperation trying to get more fresh air. If you increase how much you inhale, and are patient after you've exhaled, you can get mostly fresh air in (and possibly a bit of a high from excess oxygen as opposed to a deficit).
The plastic bottle could have some sort of gas or substance that gives off an odor. You see this sort of thing in heavy rubber scenes from time to time.
