Make the inside of that tube gag rough, so instead of the rope sliding smoothly, it moves in jerks. So even when she is trying to get it balanced, it will still jerk on her nipples when she moves her mouth.

Of course, from the way she is tilting her head, she likely wants us to cut the crotch rope first. That will allow her to sit down and rest her legs.

The rougher it is, the more likely she'll be able to wear through the rope eventually. That would defeat the purpose.
