Flea can look however they want to look. Remember that Flea is a master magician and followed you around as a bat up until you fought them in Magus' Castle.
If Flea can magically change their body to that degree, then it's probable that Fleas true form was never a trap or a woman or whatever in the first place. Heck, it's probably not even what he originally was either, he could just like looking like a woman or a very feminine man. There is no straight answer to what exactly is between Fleas legs cuz like the rest of his body, he can change it at will.
