Gelbooru has very specific policies for what qualifies for the explicit rating. You can find them here:

Basically, the explicit rating should only be used for images depicting genitals or actual sexual activities. Bare breasts do not qualify as explicit as far as Gelbooru's rating system is concerned. This is why the image is tagged questionable.
