The artist failed to realize that the barrel used isn't compatible with the M203. The rifle has a full profile barrel. The M4 uses a specially profiled barrel to allow the M203 to mount to it.
Also it's not smart to not have a hand guard over the barrel. Rifle barrels get very hot, very quickly.
