IMO I think it's a modified water wheel, except with brushes instead of the paddle-like things on the exterior (probably getting energy from another source) that were replaced with brushes to brush at her labia/clit to tickle, tease, and stimulate it.

Its not turning at all. its just two images swapping creating the illusion of movement. With a little eye work I can make the wheel move front to back, back to front, or make it so there just one brush that swipes back and forth.
