A bit of googling and the artist is fujikawa_satoshi.
The picture in question is the cover for a manga called "TS Panic".

Also that girl is actually a genderbent boy, according to the manga.
It's the usual genderbend plot: Become a girl and getfuked.
The manga is kinda large for a hentai though, around 100 pages.

This Manga isn't in english albeit there is a parallel story that is.
From the looks of it TS panic has a downer ending.

The parallel story is named Trans Panic, it has an english version online and a happy ending.

That bread has to be so limp and soggy by now, but so much more delicious too.

Also, thanks to the people who posted information about the manga, I love the ones where cute boys get changed into cuter girls.
