My impression is those are weights attached with clamps as an addition to her nipple piercings. I don't see how the testicles can be removed from her balls when she's still able to cum.

They just look like weights that have been hooked through the skin. Or the artist just forgot how to anatomy, the ballsack shouldn't look like it's got balls in it if they had been removed, right?
