The picture frame with the black ribbon draped over the top corner means he's dead. The other side of the frame would also have a ribbon, as well as a small bow in the top center.

What I think happened is, Sora got pregnant during one of the many times they fucked during the show, and if you recall at the end she almost drowns but gets saved by Haru. I think this is an alternate ending in which he dies saving her.
