Pitou is referred to with a "boyish" inflection; it is possible for it to mean tomboy so the gender is debatable in the manga. To make it easier translators used "he" for English.

Togashi has outright refused to tell people what gender Pitou is, for reasons unknown. Probably cause he's a fucking troll. This is why Pitou is female in anime - the character designers had to pick a gender themselves because Togashi wouldn't tell them.

Pitou's design is the same in both the manga and anime, so no pitou is not only female in the anime. Only the latest manag deisgn matters, not the weird early one.
