Not necessarily. A lot of pictures with bare breasts are rated as "questionable", even though they are nsfw.

It isn't a question of whether or not you or Gelbooru thinks it is NSFW, but rather whether or not you can show this to your fellow (male, & FEMALE) coworkers & EMPLOYERS and not get into trouble for it.
I know I, and the greater majority of the working class in America couldn't at our places of employment, and that's the reason why I say it's Explicit/NSFW.

My understanding is that nothing but Safe is supposed to be SFW. Questionable is nudity or sexual implications, while Explicit is sex.

Honestly you shouldn't be on gelbooru at work in any case, but the whole purpose of the Questionable category is to fill the rather significant gap between 'safe for work' and 'hardcore fucking', where everything from ecchi comedy to artistic nudity falls.

Yeah, by gelbooru.com/index.php?page=help&topic=rating the "Explicit" category is defined as 'penis or vagina clearly visible, masturbation, or penetration', "Safe" is viewable in public, and "Questionable" is anything NSFW that isn't Explicit, specifically including 'nudes, exposed nipples or pubic hair, cameltoe, or any sort of sexually suggestive pose'.
