If you know more about Saki (one of the two girls who originated from an April's Fool joke and found some unexpected popularity), this pic becomes very heartbreaking. While Hisao has the potential to live on even for decades in the best scenario, with her illness Saki is unlikely to reach her 30s and it will be a slow, painful withering until she loses even the ability to breathe.
