Maybe they only resemble the Plain Doll to the Hunter--like the Nightmare is tapping into your memories to fuck with you on a deeper level.

Lots of things in the nightmare frontier/mensis resemble things that are outside those areas. They draw from the encounters you have earlier in the game.

Not only do they resemble the Plain Doll, but their heads are many Messenger corpses smashed together.

It's thought they are rejected Dolls that were taken and twisted into things the Hunter recognizes and trusts most, to terrorize them in the Nightmare that much more (since the only place you see them is technically in the Nightmare).
