The one in the red dress is a whore, in game her blood a reward for saving her will heal you and give you a stamina buff. The one in black is a nun, whose blood for saving her is just a healing buff. If you save both of them, in the story the nun will get jealous, go crazy, murder the girl in red, and then yell at you accusing you of having tainted blood.
