She's cleaning out his ears with a cotton swab. In Japanese culture, it's a very intimate thing you only really do for people you're close to and trust. Think about it... it's easy to hurt yourself when YOU'RE doing it, so when someone ELSE does it... they need to be very careful and trustworthy.
