The reason is because Fallandia Varady, the red headed heterochromia eyed chick is a futanari in the original game.
Steam-Hearts stars the brother and sister duo Blondia aka "Blow" and Fallandia aka "Falla" Varady respectively. The idea is, there's a virus that's infecting a group of animalgirls and have driven them crazy and thus act as the game's bosses. All things simplified that's the general idea.
