It's not an anatomy fail.
It just means she has a literally 4ft long vagina where the cervix is at the bottom of the human part, since we know the vagina is at the back of the horse end thanks to another example sequence of centaur mating. Unless she has magical vacuum properties, getting pregnant must be a real chore.

But yeah, like some have stated, we are arguing about an anime anatomy failure about a mythical centaur. How geeky have we become ? Then again, that's what's make this great : it can inspire us to the point of discussing passionately about such hypotheses and details.
