You'd have to actually watch the scenes to appreciate them I guess. The other guy, Gisuke, apparently didn't penetrate her vaginally. He was saving that one for later. After rewatching several times she might've been a virgin, she briefly complained about the pain in between the panting and screaming. Also while Tokiko is the protagonist's elder sister it appears that she works as his servant, so most likely she was the daughter of a mistress of Fuuma Danjo, the protagonist's father.
