Well, given on pixiv the image was tagged "男の娘"("otokonoko"), which from what little I can gather from the tag encyclopedia for pixiv's tags means something like 'trap' and 'femboy'..
I think these two are supposed to be dudes? ...Anyone with better knowledge of japanese wanna dig around on pixiv and check?

One's the protagonist from Day of Revolution/Kakumei no Hi (who also appears in Princess Princess), they are intersexed, ie, were genetically female but expressed masculine anatomy initially and eventually began to transition to female. The other one is the protagonist of Pretty Face, who is completely male but due to stupid plot aspects gets his face reconstructed into a female's and winds up living as a female for an extended period of time while still being mentally male. It's been a long long long time since I read it, but I believe he got extremely realistic falsies that could attach to his chest for a certain point, which is probably what's going on here.
So, ultimately I'd argue Masashi counts as a trap while Kei/Megumi is a bit more complex but would probably qualify as a newhalf from an anatomical perspective since I don't think there are really tags for forms of intersexuality beyond true hermaphroditism, which generally gets categorized as futanari here, which isn't appropriate.
