The box is designed to send electrical current through the wires coming out the side when the circuit is complete. Those wires are connected to the girl, giving her a (likely painful) shock. Said circuit is connected to a wire attached to the cat toy; when the wire touches the metal plate on top of the box, it shocks her. The toy is on a spring to pull it back upwards, but whenever the cat attacks the ball, it pulls the toy down, completing the circuit.

So, the more the cat plays with the poofy ball, the more she gets shocked.
