Generally not, at least with real breasts - those cranked-way-high implants might be a different story. Real breasts are mobile enough that a doctor can nudge them aside to get the stethoscope where he needs it to be. This image isn't a good example, tho - he should have that thing flat on her sternum, not sitting right on her tit like that. Also she wouldn't have to be topless for him to check a heartbeat.
...You know, I'm beginning to think this doctor is kinda sketchy.
