Yes, they need to get a bit more creative, this has particular occurrence has become too cliche.
Heh, at least To love-Ru took this cliche to it's near absolute extreme.
Is there a reverse? Is there any anime where the female and male characters fall and the female ends up accidentally with her hand on the guys crotch?

That sort of thing happens sometimes - usually with her just falling on him into a pseudo-intimate position, but of course never with her hand anywhere truly intimate. But even when it's reversed, the "punchline" is exactly the same - the female turns red, gets angry, the male pales, babbles excuses, the female screams, hits the guy, blah blah blah.
I'm honestly curious if there's something in the Japanese culture that values familiarity over novelty in its comedy, because I legit cannot understand how this crap is so common otherwise.

But most of the fanservice back that was, at best, either a panty/upskirt shot or Barbie doll nudity. And even then it was play more as a fact-of-life. The accidental groping gag is more of a modern thing and is very, very over used.
