The hymen shouldn't be ruptured. It's an elastic membrane that is designed to stretch. The girl needs to be lubricated, and it helps if she or her partner widen the opening with their fingers prior to penetration. Rupturing only occurs if the dude doesn't know or doesn't care that he's supposed to be careful. (Or if she's got an imperforate hymen, but that's rare and requires medical attention anyway.)
