I'd say with very little optimisism, to people who haven't played Undertale they could POSSIBLY mistake Mettaton EX for a girl and so the "he/him" bit is to clarify them, but again thats what little of my optimisim for the human race showing, most likely put in due to the artist also being sick as shit of all the "DUURRRR ITZ A TRANNEH!!!! CUASE MAH HEADCANCER!!!!!!", the ONLY transitioning Mettaton did was from ghost to metal body, so please tumblr GTFO with that transgender headcanon-cancer crap

Pretty much what anon above me said. For some reason, people mistake Mettaton for a female a lot.
