Are the Doll's hands just one more pair of hands dragging the Hunter to hell... or are they the only thing keeping the Hunter from drowning?

I like to think that it plays on the use of Insight. What is noticeable is that there are different kinds of hands on the Hunter's body, even a snake is coiling around his leg. One of those hands (wrapped around the Hunter's right arm) is actually human in comparison to the long nails/claws of the other figures.
