It was Shion who went psycho bitch, not Mion, at least in the anime. I don't know about the game though.

Yeah it's the same in the visual novel. The Hinamizawa syndrome is triggered in Shion the moment Keiichi gives that damn doll to Rena... rest as they say is history.
