People are ignorant. She doesn't have balls is fact. If she's intersex or transgender and underwent female hormone treatment her penis would've shrunk she can't even get it up because of it and she'd eventually go full female.
Poison however was originally female but America (or other countries too) didn't think it was polite to hit women in games unless they were transgendered or whatever because they wouldn't be male or female identifiable. That's how the intersex thing began and the retarded degenerate comments started. She's not queer the slightest or transgendered, she's the product of stupidity, albeit not being a stupid character.

...but her original concept sketchs and art from before the game was released has "new-half" by her name. She was always meant to be that way. They didn't "change it" to suit the west.
Other very popular beat-em-up games at the time had you bashing girls too. Double Dragon in particular. So them changing it wouldn't make any sense either way.
The only time with certainty we can say they changed Poison in ANY way was on some early home console ports, where she was completely replaced with two generic looking thugs in the SNES version and in the Megadrive/Genesis version had her hit/damage sprite altered to show less cleavage, to name a couple.
