Theory of mine:
The belt over her crotch has a hole in it.
There is probably some sort of inflexible reinforcement around the edge of the hole or the belt is just really tight or something.
So since the costume is such a tight fit for the waist, they can't move it enough to pull the dildo out, and the base of the dildo does not fit through the hole.
... or just go with "it is stuck there with magic"
