I think this pictures is based from GachGacha Secret. Which is about a guy who is in an accident with a virtual reality game called Gacha in which he gains the power through Sneezing to change back and forth from man to girl and back to man, in girl from he looks exactly like the small girl in this picture. when in this form how ever if he eats Nattou his girl form grows up to look just like the mature woman here, in an instant of course. He uses this power to get close to a girl he loves. its a fun Manga and very Ecchi. The main Character is called Akira-kun/Akira-chan. that is assuming thats who the artist drew here
