This happens to escapees when captured by Glados. They are forced into a body straitjacket, and hung upside never to escape imprisonment again.

It is Glados, if you look at her model in the game it looks like a women hung upside down. If you don't believe me check the half life wikia

I recall reading (though, don't take my word for it) that in actual fact, Glados is the mind of the late Caroline, who was PA within the Aperture Science Center. The concept of Glados (being a Genetic Lifeform and Disk Operating System) was that the original Founder and CEO of Aperture Science Enrichment Center, an individual named Cave Johnson was to have his subconsciousness uploaded into the System (due to an illness, which resulted in him slowly dying). Having passed on before the Glados 'Project' could be finished, Caroline took over the Aperture Center Role for Johnson (and was supposedly) forced into becoming Glados, by having her mind uploaded into the System. The result left the Glados / Caroline mentally unstable, which resulted in her attempting to (and had) killed Employers within the Center.

This picture was drawn to make the case, that GlaDOS was modeled to resemble a woman hung upside down, which was dismissed by one of the designers. The rumor stems from the fact, that at one stage in the design process they had the idea to model her after the Venus of Milo upside down which they didn't use in the end.
