Sure he does, it's just still in the sheath. It's that bulge between his legs. Horse peni (plural for penis) expand quite a bit when the horse gets aroused. Of course, the artist didn't draw an opening for said sheath, so it's a little hard to tell.
Also, baby horses (aka foals) are pretty big. If she's in the late stages of pregnancy, this is actually a fairly realistic size belly. You know, assuming she could get pregnant in the first place.
So there's your piece of weird knowledge for the day.
