I have a friend who nosebleeds with high blood pressure of all kinds, at random. Not every time, but happens plenty of times.

I think it started as a non-ecchi replacement for instant erections. Later they added girls nosebleeding for comedic effect. The first time I saw it was in Tenchi Muyo! Ryo-Ohki back in 1993.
