The right nipple looks high up, but breasts, REAL breasts do flatten out when the girl lies down. They're generally fatty tissue, so what do you expect, them to stand erect like towers? Fake breasts will stay perfectly round ... depends on the quality of the fake breast, of course. Girls who get the ultra-round fake breasts will look like they have round mounds even when they lay down, and the skin can look crinkled on the outline of the breast ... not very attractive. A good breast job will fill the breast out, but still look natural, which includes flatenning out when lying down. That's why half-filled saline implants are good...they really enhance the breast. If a girl doesn't have much breast to begin with, and goes up quite a few sizes, it'll just look awkward. But if she has a decent breast and just enhances it a cup size or two, then it can still look natural. As for this pic, the whole sketchy lines of it just seem tacky. The coloring is ok, but the line work degrades. Her face is nice, though. Love a girl in the throws of ecstasy.

Good implants are usually done behind the pectoral muscle, thus pushing it out along with the natural breast tissue. Unfortunately, this can cause complications if the girl performs strenuous exercise. The next best thing is to hid the implant in behind the natural breast tissue, but in front of the pectoral muscle. Using a soft saline implant, this will help fill out the breast tissue and still look natural. Unfortunately, most girls don't go for a size bigger. Most getting boob jobs go for 2-3 sizes bigger, and most are butt-skinny girls with A cups to begin with. So, the doc has very little breast tissue to hid the implant in. And hence you get the super-mounds that most porn stars and strippers have. But, it's better seeing that then some crinkly-looking, deflated boobs from a girl who's had several kids.
