One very cool rule of art is that you can forego elements of realism to emphasise certain features or enhance the very message of the piece. In this case, the penis is made to look as unrealistically bendy and grotesque as possible - like a 'vomiting pink snake' - to serve as a stark contrast to Komi's serene beauty.
Breasts of large enough size will usually hang this way when a woman leans far forwards like Komi does in this pic. The artist chose to keep the realism in this case - despite the unfortunate association with geriatric anatomy - presumably because it's hot as fuck. The 'droop' reminds you that breasts are soft, malleable playthings for men. This, her amazingly-drawn lower body, even the weird snake dick, they're all intended to remind you this: that a woman's body is an inherently sexual property, that we will always be enticed by it, that we will never stop fapping to the very idea of it.
I recommend the sagging_breasts tag for more and hopefully you can see the appeal. I remember the artist Moisture_(chichi) had a really popular Rosa_(Pokemon) piece with really weighty sag. Sorry for the autism.
