Nobody here seems to know their Capcom lore, so I guess I'll clean up this shit-storm of wrong answers...
Poison started off as a basic female enemy in Final Fight, but this was quickly changed by Capcom in order to avoid any possible negative images concerning violence towards women by claiming she was a man.
Recently, it was made clear by Yoshinori Ono (current producer of Street Fighter 4), that Poison is officially a post-op transsexual to the US market and a pre-op transsexual to the Japanese market.
That's how the story goes according to Capcom.
...Of course, I prefer to believe Poison is always packing cock no matter where she is, but that's just me.
