Time to over-analyze hentai.
The kid may be trans. The bottle label is cut off, but the letters "adiol" can be read, so it's probably estradiol (a feminizing hormone). The pills are long and blue, and estradiol pills are usually either long and blue or round and white. Menhera Chan (the drawing of the girl above the wojak drawing) is often used in the trans community as the subject for memes and whatnot (usually transfem). Their arms are cut, possibly due to shaving in order to achieve a more feminine look. The only real piece of conflicting evidence is the word "trap" written on their ass backwards. Some trans girls do reclaim that word for themselves, but most view it as a slur (I'm not going into why or if they're right to do so, do your own research). There may be more pieces of evidence for and against that I didn't pick up on. Of course, it's also entirely possible they're just a very feminine femboy who takes hormones in order to appear even more feminine. At the end of the day it doesn't matter in the slightest.
In writing this, I noticed that there are two names in the image. Robin on the nametag, and Reiko(?) on their ass. A Google search for reiko turned up a character from mortal kombat and a character from bnha. Reiko appears to be a masculine name, while to my knowledge robin is gender neutral. Huh.

Reiko's a feminine name (when taken in japanese context.) I think MK blundered that one like Ray-Den vs Rye-Den.
