Looking again I still think its the same person. If you expand the image you can see the same ring on her left hand.

The woman is the same in both pictures. The old man is the woman's father in law. Both the image's title and commentary on pixiv make reference to "father in law" "three generations of a family" and "the woman who overlaps the skin". Furthermore, if you look closely behind the old man's ass you'll see a sapling; now look behind the boy's ass: the sapling is now an adult tree. Also note that -at the time of writing this comment- the translation of the left panel's title is not complete.

Let me get this straight. The one on the right is a woman getting konck up by her fater (in-law).
The one one the left is the same woman (with a different hairstyle it seems) getting knock up by her son.
So I must assume the son's father is also his grand father?
Than, the next child to be born will be both the son's son/daughter and brother/sister at the same time.

His dad is his grandpa, so his mom is his sister, so he is his own uncle. So if he knocks up his mom/sister and she has a girl, that kid will be his daughter, his sister (daughter of his mom), his niece (daughter of his sister), his cousin (daughter of his uncle), and his aunt (sister of his uncle). To the mom, the kid is her daughter, her granddaughter, and her niece.
