Anon5, yes there are cases like that in reality, just very rare and not very publicised. Usually if this happens the parents ask for surgery to be done to take it off, leaving the baby as a regular female. In these cases, its usually just an extension of the urethra, with all the female parts still there, so it's easy to fix.
Either that or the parents dont care and you get a kid like this who probably get seriously messed up in the head later on in life.

and @anon4, Futa must have a vagina and a penis. The Japanese word itself can be roughly translated to mean "two parts." If there's just penis+breasts (like in The Amanoja 9's works) then it's just a trans or "newhalf" as they call them in Japan.
