Why do so many people tag small_breasts as flat_chest? Isn't flat_chest supposed to imply something flat and not just a little below average? (And I feel I must mention that I'd reckon these breasts in particular are actually closer to medium sized than flat on the sliding scale of small breasts.)

The simple answer is that "small_breasts" is a tag that was created here on Gelbooru. Danbooru doesn't use the tag, and instead aliases it into flat_chest. Additionally, "medium_breasts" isn't even something that they see as a valid concept over on Danbooru; their reasoning is that if breasts are too big to be flat_chest, but not big enough to be large_breasts, you don't need an extra tag to fill in the distance since it's a gap that's covered by "breasts -large_breasts -huge_breasts -gigantic_breasts".
Since 70% of our uploads come in from Danbooru, along with their tags, it stands to reason that this would make the small_breasts tag not properly used. Ultimately, it would probably be a good idea for us to switch to a similar system, and it's been proposed a couple times, but nothing's ever come of it.
tl;dr it isn't actually a person tagging these, but rather the automatic importer bot, so there's nothing anyone can really do other than fix them if they find them.
